1. Locate a provider
You love choices - and so do we. That's why our network has thousands of independent doctors and retail providers. Visit eyemed.com to find a provider in the Advantage network near you.
2. Schedule an appointment
Online appointment scheduling is available on eyemed.com. You can also call ahead or stop by one of the many providers that offer walk-ins. Most also have evening and weekend hours to fit any schedule.
3. Show your ID card
Download and print this ID card. When you arrive, let the provider know you have a discount and show them the card.

Eye exams available from independent doctors of optometry next to LensCrafters, Pearle Vision and Sears Optical in most states. Most doctors are independent. Doctors in some states may be employed by LensCrafters or Pearle Vision in some locations.
In California, optometrists are not employed by LensCrafters or Sears Optical, which do not provide eye exams.
For LensCrafters, eye exams are available from optometrists employed by EYEXAM of California, a licensed vision care service plan. For Sears Optical, eye exams are available from self-employed doctors who lease space inside the store
